Description

Hours: Part time/20 hours; evening Shift - 4pm- 9:30pm Monday - Thursday or Tuesday - Friday.

 

  • Independently assess, coordinates and implements a timely, safe patient discharge plan to the appropriate level of care.
  • Participates in interdisciplinary approach to patient care management.
  • Works with complex patient populations who have high acuity chronic illnesses.
  • Develops treatment plans, provides patient and family education and serves as a professional resource.
  • Provides emotional support and psychosocial counseling to patients. 


MINIMUM QUALIFICATIONS: 

  • Masters in Social Work from an accredited Institution.
  • At least 2 years of relevant social work experience preferred.
  • Experience in an Acute Care setting or post-acute care setting desired but not required.
  • Positions in this classification require LMSW license in the state of Georgia.
  • Must have working knowledge of software applications.
  • ACM, CCM preferred.
