Description

We're seeking a dedicated Social Worker / Clinic LMSW to continue the growth of our Social Worker Team at Emory Northlake. As part of our team you will be responsible for independently assessing, coordinating, and implementing a timely, safe patient discharge plan to the appropriate level of care. You will participate in interdisciplinary aproach to patient care management. You will work with complex patient populations who have high acuity chronic illnesses by developing treatment plans, and establishing family educational services as a professional resources. 

 

Other desirable transferable experience: solid organ transplant experience, hospice experience, case management experience with community agencies or medical clinics (sickle cell, hemophilia, HIV, DFCS/foster care with medically fragile children, dialysis, etc)

Less ideal and less transferable experience: only having acute inpatient psychiatric (Peachford, Grady Psych, Ridgeview) , only having counseling experience, MSW only

 

MINIMUM QUALIFICATIONS:

  • Masters in Social Work from an accredited Institution.
  • At least 2 years of relevant social work experience preferred.
  • Experience in an Acute Care setting or post-acute care setting desired but not required.
  • Positions in this classification require LMSW license in the state of Georgia.
  • Must have working knowledge of software applications.
  • ACM, CCM preferred.
