Description

8am - 4:30pm / Full Time/ 40 Hours 

 

We're seeking a Transplant Coordinator- LPN, who role is an outpatient position that focuses on overall health and population management of a cohort of patients. The LPN Transplant coordinator may work with pre-transplant, post-transplant, or living donor patients.

 

  • Ordering and following labs or imaging studies per practice routine with standing order per organ group protocol.
  • Communicate normal results.
  • Communicate abnormal results per provider written guideline- after/based on provider review.
  • Triage in basket and pool messages per clinic specified guidelines and security settings, review results per clinic specified guidelines and security settings, and coordination of care across settings.
  • Facilitate care management of defined populations.
  • Coordination of information between the physician and patients. Functions under direct supervision of the licensed provider or RN. Demonstrates mastery of departmental competencies.

 

MINIMUM QUALIFICATIONS:

  • High School Diploma or GED required.
  • Graduate of an accredited Licensed Practical Nursing Program. 
  • Three (3)  years patient care experience
