Description

8:30a – 5:00p / Full Time / 40 Hours 

The Director of Pharmacy for Population Health is responsible for leading and managing the pharmacy services within the population health programs. This role involves strategic planning, directing, monitoring, and evaluating the provision of pharmacy services to ensure optimal system and plan performance in assigned business areas.

Key Responsibilities:

  • Provide managerial and leadership support for the pharmacy's population health programs and staff.
  • Develop and implement strategies for achieving quality and population health performance goals.
  • Collaborate with healthcare teams to promote patient-centered care and integrate pharmacy services into broader health initiatives.
  • Oversee the management of medication therapy in a cost-effective and evidence-based manner.
  • Utilize data and analytics to inform decisions and improve the health outcomes of the population served.
  • Implement processes to monitor, track, and measure specific treatment related interventions and outcomes.

Minimum Qualifications:

  • Doctor of Pharmacy (PharmD) degree from an accredited institution.
  • Current pharmacy licensure in state of Georgia (or eligible).
  • Five years of experience in clinical pharmacy practice and management, preferably in a population health or managed care setting.
  • Strong leadership, communication, and interpersonal skills.
  • Demonstrated ability to work collaboratively with multidisciplinary teams.
  • Knowledge of healthcare systems, quality improvement, and population health management.

 

Preferred Experience:

  • Master’s degree in pharmacy, health services, business administration, or related field preferred.
  • Experience with healthcare analytics, value-based care and reporting tools.
  • Familiarity with public health principles and the social determinants of health.
  • Proven track record of implementing innovative pharmacy programs that improve patient outcomes.
