Description

The Medication Reconciliation Assistant (MRA) practices within the health system and works under the supervision of a pharmacist to collect and document the best possible medication list, including the correct drug name, strength, route, and administration frequency for all current prescription and over-the-counter medications.

 

Shift Time: 11a-11p

  • The MRA will interview patients, their families, external medical providers/facilities, and/or outpatient pharmacies to confirm the accuracy of the medication list.
  • The MRA coordinates corrections to a patient's medication list by notifying physicians and/or pharmacists of updated information.
  • The MRA may obtain, add, or correct medication allergy information including the medication name and type of reaction.
  • Under the general supervision of a pharmacist, the Medication Reconciliation Assistant is responsible for the collection of the best possible medication list.
  • The process of collecting this information may include interviewing patients, their family members/caregivers, external medical providers/facilities, and outpatient/mail-order pharmacies.

MINIMUM QUALIFICATIONS:

  • Completion of at least two years of college.
  • Successful completion of an ASHP-accredited technician training program or enrollment in a professional-related school preferred.
  • 2 years of experience as a Pharmacy Technician.
  • Current CPhT certification by passing the CPhT national exam or currently enrolled in accredited School of Pharmacy with current Georgia Pharmacy Intern license.
  • Current Registration with the Georgia Board of Pharmacy.
  • Must maintain certification and registration through entire length of employment.
