Description

We're seeking a dedicated for either a MSW / LMSW/ LCSW Social Work Case Manager to continue the growth of our Social Work Case Management team at Emory Hillandale Hospital. As part of our team you will be responsible for coordinating clinically complex patient care needs across the health care continuum.

 

Our Social Worker will assess patients for transition needs including facilitating high quality clinical and cost outcomes; negotiating, procuring and coordinating services and resources needed by the patients in a timely and effective manner.

 

MINIMUM QUALIFICATIONS

  • Masters in Social Work from an accredited Institution.
  • At least 1 year experience preferred, experience in an Acute Care setting preferred.
  • Positions in this classification require MSW license or license eligibility in the State of Georgia.
  • Knowledge of software applications. ACM, CCM preferred.
  • For Behavioral Health job locations, Master's degree in Psychology, Counseling, or related field from an accredited institution will be accepted in lieu of Master's degree in Social Work and license eligibility for LAPC/LPC in the state of Georgia will be accepted in lieu of license eligibility for LMSW to meet the minimum job requirement.
