Description

Diabetes Educator

Shift: Full Time 8am-4:30 pm

Location: The Emory Clinic, 1365 Clifton Road Atlanta, Georgia 30322

 

  • Functions as a member of a multi-disciplinary team that manages the care of the diabetic patient population, requiring a high degree of specialization in nutritional skills and knowledge.
  • Provides nutritional care to patients including assessment, counseling, education, evaluation and management. Develops, conducts, and evaluates patient and family education by discussing the plan of care.
  • Determines appropriate interventions for identified patient problems, symptoms, behavioral changes and teaching needs. Documents all aspects of patient care.
  • Communicates effectively with patient, family and members of the health care team.
  • Administers clinic policies and procedures in accordance with state, federal, and organizational guidelines at each institution; may assist in reviewing policies and procedures.
  • Determines and mobilizes equipment and resources required for effective patient care.
  • Serves on Clinic committees. May supervise support staff. Develops and prepares operational and statistical reports for management and regulatory agencies to assist in making fact-based decisions and to comply with regulations. 


MINIMUM QUALIFICATION

  • Registered nurse or Registered Dietitian in the state of Georgia.
  • Certified as a Diabetes Educator
  • Two years of related experience.
